Aster Clinic & Pharmacy (referred to in this policy as “Aster”, “we”, “us”, or “our”) is committed to protecting the privacy and confidentiality of the personal and health information entrusted to us. This Privacy Policy explains how we collect, use, disclose, retain, and safeguard your information, and it describes the rights available to you. We handle your information in accordance with applicable law in the Province of Alberta, including the Health Information Act (Alberta) (the “HIA”), the Personal Information Protection Act (Alberta) (“PIPA”), and, where applicable, the federal Personal Information Protection and Electronic Documents Act (“PIPEDA”).
1. Scope of this Policy
This policy applies to information collected through this website, in person at our clinic and pharmacy, and through any other interaction you have with us. As a custodian of health information under the HIA, and as an organization subject to PIPA, we are responsible for the information in our custody and control, including information transferred to third parties that provide services on our behalf.
2. Information We Collect
We may collect the following categories of information:
- Identifying information, such as your name, date of birth, address, telephone number, and email address.
- Health information, such as your Alberta Health Care Insurance Plan number, medical history, symptoms, diagnoses, prescriptions, medication records, laboratory results, and other information necessary to provide care.
- Billing and insurance information, such as private insurer details and information required to process claims and direct billing.
- Website information, such as limited technical data (for example, browser type and pages visited) that may be collected automatically to operate and improve the website. Any appointment booking is completed through a third party booking provider, and information you submit there is governed by that provider's privacy practices.
3. Purposes and Use
We collect, use, and disclose your information only for purposes that a reasonable person would consider appropriate in the circumstances, including to:
- provide, coordinate, and deliver medical and pharmacy services;
- dispense medications and conduct medication reviews;
- communicate with you about appointments, results, and follow up care;
- process billing, insurance, and payment;
- maintain accurate health records; and
- meet our legal, regulatory, and professional obligations.
We do not sell your personal or health information.
4. Consent
We obtain your consent to collect, use, and disclose your information, except where collection, use, or disclosure without consent is permitted or required by law. Depending on the sensitivity of the information, consent may be express or implied. You may withdraw your consent at any time, subject to legal or contractual restrictions and reasonable notice, although doing so may affect our ability to provide certain services to you.
5. Disclosure to Third Parties
We disclose health information only as permitted or required by law. This may include disclosure to other treating health care providers involved in your care, to Alberta Health and Alberta Health Services, to our authorized service providers under appropriate confidentiality obligations, or where required by a court order or by law. Where we engage service providers, we require them to protect your information in a manner consistent with this policy and applicable law.
6. Safeguards
We maintain reasonable physical, administrative, and technical safeguards designed to protect your information against loss, theft, and unauthorized access, use, modification, or disclosure. Access to health information is limited to authorized personnel who require it to perform their duties.
7. Retention
We retain your information only as long as necessary to fulfil the purposes described in this policy and to meet our legal, regulatory, and professional record keeping obligations, including those established under Alberta health legislation and by our regulatory colleges. When information is no longer required, it is securely destroyed, erased, or made anonymous.
8. Your Rights: Access and Correction
Subject to limited exceptions permitted by law, you have the right to request access to the personal and health information we hold about you, and to request correction of information that is inaccurate or incomplete. To make a request, please contact our Privacy Officer using the details below. We may require verification of your identity before responding, and we will respond within the timelines required by applicable law.
9. Concerns and Complaints
If you have a concern about how your information has been handled, please contact our Privacy Officer first so that we may address it. You also have the right to contact the Office of the Information and Privacy Commissioner of Alberta, which oversees compliance with the HIA and PIPA.
10. Changes to this Policy
We may update this Privacy Policy from time to time. The revised policy will be posted on this website with an updated effective date, and your continued use of our services indicates your acceptance of the changes.
